Renewal Cards from the State are taking several months to receive. We recommend you apply for your renewal 60 days before your card expires. So give yourself time in advance! Schedule yours here.

If it has been almost 1 year since your certification appointment, you need to come in for an annual follow up with the doctor. It is required by the state to have an ongoing physician/patient relationship. Schedule yours here.
